Formfield in modular page does not appaer

Hi,
I used this instruction:

to set up a contact form on my modular webpage which uses the antimatter theme.
Unfortunatly the formfied doesn’t appear. The path of my contact directory is: /home/_kontakt
In that directory you can find the form.md and the modular.md.

This is my form.md

I can’t figure out where i did the mistake.
Hope someone can see where it is.

The structure must be

/home/
/home/modular.md
/home/_kontact/form.md

Is that what you have?

Thanks for your answer i figured it out. I had both files form.md and modular.md in the same Folder
